Please submit the below form and our team will be in touch to arrange. Be advised that our hub provides a drop-off facility only. Samples must arrive at our central Bristol clinic within 45 minutes of ejaculation. You will be required to pay for your test once a sample collection date has been agreed. A payment link will be sent to the email address provided. If payment is not received within 10 working days of the sample collection date the booking will be cancelled.

If you have had any of the following then a sperm DNA fragmentation test should be considered:
Recurrent miscarriages or chemical pregnancies.
Inability to conceive or a diagnosis of 'unexplained infertility'.
Embryos that fail to develop 72 hours after fertilisation.
Morphology (normal forms) of 4% or below.
Varicocele (varicosed veins in or around the scrotum)
